Dual Roster Form

Players who intend to dual-roster must complete this form.

If you are entering into Dual Roster Agreements between multiple associations, please fill out this form and send it to president.vyha@gmail.com for signature. The President will return the signed form to you or, if it is fully executed, forward it to the Central Section for approval. 

Players cannot skate until the Central Section approves the Dual Roster form.

All signing parties acknowledge:

1. The Player is committed for the current season and has an obligation to attend all “Current Association” games and practices in the event of any scheduling conflicts.
2. All financial obligations must be paid in full to “Current Association”.
3. Agreement is not considered a Player Financial Release.
4. The signed agreement must be submitted to the Section President for approval.

Valley Only Players who intend to dual-roster must complete this form.

If you are a current Valley player entering into Dual Roster Agreements between multiple Valley teams, please complete this form and send it to president.vyha@gmail.com for signature.

Players cannot skate until the President approves the Dual Roster form and the dual roster fee is paid. Participating with the secondary team before payment will void this agreement for the entire season.

All signing parties acknowledge:

1. The Player is committed to the current season and is obligated to attend all “Primary Valley Team” games and practices, even if there are scheduling conflicts, unless previously agreed to by the primary team's coach.
2. Players will be required to pay the dual roster fee.
3. The agreement is not considered complete until the dual roster fee is paid to Valley Youth Hockey. Participating with the Secondary team prior to payment will void this agreement for the entire season.
4. The signed agreement must be submitted to the President for approval.
